ADULT-GERONTOLOGY NURSE PRACTITIONERMelissa has more than a decade of experience caring for older adults in primary care, home-based medicine, assisted living, family medicine, and nephrology, including caring for kidney transplant patients. Before becoming a nurse practitioner, she spent many years as a registered nurse caring for medically complex patients in acute care.
She believes exceptional healthcare begins with listening, building relationships, and providing thoughtful, personalized care that helps patients remain healthy, independent, and comfortable at home.
BOARD-CERTIFIED PHYSICIAN ASSISTANTTim Francis, P.A.-C
Tim is a board-certified Physician Assistant with more than 20 years of experience in primary care, hospital medicine, orthopedics, pain management, and geriatrics. As the former owner of Senior Helpers of Connecticut, he also spent years helping older adults and their families navigate the often-complex healthcare system.
His passion is building long-term relationships with patients, coordinating care across providers, and helping older adults maintain their health and independence through compassionate, home-based primary care.

Our vision is to create a seamless model of home-based care that empowers patients to age safely in place and supports caregivers at every step. We aim to deliver accessible primary care and coordinated services that guide individuals through every stage of their health journey, bringing high-quality, compassionate care directly to those who need it most in the place they call home.
We coordinate care using the major hospital record systems in Connecticut, including Yale New Haven, Hartford HealthCare, and Griffin Hospital.
