Meet Melissa, Co-Founder of Guiding Light House Calls
Melissa Francis is a board-certified Adult-Gerontology Nurse Practitioner specializing in complex medical care for older adults living at home. Her work focuses on patients who often struggle within the traditional healthcare system such as those with mobility limitations, multiple chronic conditions, or cognitive decline who find it difficult to access consistent medical care.
Melissa began her healthcare career in 2004 as a home health aide, where she saw firsthand how difficult it can be for vulnerable patients to access reliable care. While earning her Bachelor of Science in Nursing from Florida Gulf Coast University, she worked as a dialysis technician, gaining early experience managing chronic and complex illness.
As a registered nurse, Melissa spent seven years in acute care medical-surgical nursing before advancing to specialized roles in cardiothoracic surgery, LVAD, and solid organ transplant at the Texas Medical Center in Houston (one of the most advanced medical environments in the country). There she witnessed both the extraordinary capabilities of modern medicine and the consequences when quality of life and patient-centered goals are overlooked.
That experience fundamentally shaped her approach to care.
Melissa went on to earn her Adult-Gerontology Primary Care Nurse Practitioner degree from the University of Texas and has spent the past decade practicing across urgent care, primary care, assisted living, and home-based medicine. Now, she focuses almost exclusively on caring for older adults living at home.
Many of these patients are medically complex, and are among those least well served by the traditional healthcare system.
Through her house call practice, Melissa provides comprehensive primary care directly in the home, eliminating the need for difficult and disruptive office visits. She partners closely with patients, families, and caregivers to coordinate care, manage chronic conditions, prevent avoidable hospitalizations, and guide medical decisions with clarity and compassion.
Her goal is to help patients maintain dignity, independence, and quality of life while giving families the reassurance that an experienced clinician is guiding their care every step of the way.

We aim to deliver accessible primary care and coordinated services that guide individuals through every stage of their health journey, bringing high-quality, compassionate care directly to those who need it most in the place they call home.
We coordinate care using the major hospital record systems in Connecticut, including Yale New Haven, Hartford HealthCare, and Griffin Hospital.
